These Terms & Conditions (“Terms”) govern participation in the Specialty Care Services (“SCS,” “we,” “us,” “our”) Referral Program (“Program”). By submitting a referral or participating in the Program, you (“Participant,” “you,” “your”) agree to these Terms.
Plain-English note: This Program exists to encourage high-quality introductions through existing relationships while maintaining ethical standards and compliance.

Program Overview
The Program allows eligible Participants to receive a referral reward when they refer a new client, “Referred Client,” who starts and completes a minimum period of paid, eligible services with SCS. The Program may apply to SCS home care services and, if launched, future cleaning services (as determined by SCS).
Who Can Participate
Participation is open to individuals and organizations permitted by law and applicable regulations, subject to SCS approval.
SCS may refuse participation or deny a reward if participation would violate law, payer rules, ethical standards, or create a conflict of interest.
Eligible Referrals (What Counts)
A referral is eligible only if all of the following are true:
- New client: The Referred Client is not an active SCS client at the time of referral and is not already in an active intake/onboarding process with SCS.
- Identified at intake: The referral must be identified during intake (or as otherwise approved in writing by SCS).
- Services start: The Referred Client starts services with SCS.
- Services are paid: SCS receives payment for services (not merely an authorization).
- Meets the qualification threshold: The Referred Client remains active and completes the minimum paid service threshold described in Section 6.
- Meets payer eligibility: The Referred Client is an eligible payer type (see Section 5). Not eligible: leads, assessments, consultations, authorizations only, “they called us,” or unpaid services do not qualify.
Referral Submission Requirements
To submit a referral, you must provide accurate contact information and confirm you have permission to share the Referred Client’s details with SCS.
You agree not to:
- Submit false or misleading information
- Harass, pressure, or spam the Referred Client
- Misrepresent SCS, Program terms, or services
- Use the Program in a way that violates any law or regulation.
Payer Eligibility (Critical)
Rewards are available only for referrals that become paying, non-government clients, including Private Pay and other approved non-government payer sources.
Excluded under any circumstance: Medicaid waiver, county-funded, and other government-funded clients are not eligible for referral rewards.
Payer changes: Eligibility is determined based on the payer source during the qualification period, not future or retroactive payer changes.
Qualification Period (When a Reward Is Earned)
A referral reward is earned only after SCS confirms the Referred Client has completed:
- Minimum paid service period: [30 days] of paid services, and/or
- Minimum paid service hours: [168 hours] of paid services, as determined by SCS policy and documented in writing for the Program.
Standard reference: SCS commonly uses 30 consecutive days of paid services unless otherwise specified in writing by management.
SCS may also require eligibility only after receipt of payment for the first 30 days or after the first 168 hours of service.
Reward Type, Amount, and Limits
Reward amounts and tiers:

Non-cash rewards: Rewards are typically issued as service credits or approved non-cash rewards. SCS may restrict available reward types, including switching between service credits and gift cards.
Limits:
- One referral reward per Referred Client (no stacking).
- Rewards are non-transferable, not redeemable for cash unless explicitly stated by SCS in writing.
- SCS may impose caps per Participant per year or per household.
“First Referral Wins” and Duplicate Referrals
If more than one Participant refers the same Referred Client, the reward is credited to the Participant whose referral is first received and documented by SCS (based on timestamp and intake confirmation), unless required otherwise by law.
SCS’s records are the final authority for determining first referral.
Review, Approval, and Timing of Payouts
Rewards are issued only after:
- Intake documentation review,
- Billing/payment confirmation
- and Management approval.
Rewards are reviewed and issued on a scheduled basis. Same-day or ad-hoc payouts are not permitted.
SCS may delay or deny rewards if documentation is incomplete or eligibility cannot be verified.
Disqualification (Why a Reward Can Be Denied)
SCS may deny or revoke a reward if:
- The referral is government-funded or becomes ineligible during the qualification period.
- The Referred Client does not meet the paid service threshold.
- The referral was not identified at intake (or approved in writing).
- Fraud, manipulation, or false statements are suspected.
- The Participant violates these Terms or any applicable law/regulation.
- Paying the reward would violate anti-kickback, referral fee, insurance, or healthcare regulations or create a conflict of interest.
Taxes and Reporting
You are responsible for any taxes related to rewards. SCS may issue required tax forms (e.g., 1099) if applicable, based on total rewards and legal requirements.
Privacy, Consent, and Communications
You must have permission to provide the Referred Client’s contact information to SCS.
SCS will handle personal information according to its privacy practices and applicable law. SCS may contact the Referred Client using the information provided for intake and service discussions.
Program Changes, Suspension, or Termination
SCS may modify, suspend, or terminate the Program at any time, with or without notice, where permitted by law.
Changes apply prospectively; however, eligibility determinations and payout timing remain subject to verification and compliance requirements.
No Guarantee of Service or Acceptance
Submitting a referral does not guarantee that SCS will accept the Referred Client, that services will start, or that services will remain active.
Client acceptance depends on clinical appropriateness, staffing availability, service area, payer eligibility, and other operational factors.
Disputes and Final Decisions
SCS will review disputes in good faith, but SCS’s determination is final regarding eligibility, payer qualification, and payout approval, to the extent permitted by law.
Governing Law
These Terms are governed by the laws of the State of Maryland, without regard to conflict-of-law rules.
