Who We Serve 

We support adults with life-impacting neuropathy from any cause. The most common reason families call us is because they’re experiencing severe and painful neuropathy symptoms with a fear of amputations, and need our assistance with neuropathy treatments and prevention. Our patients often experience: 


  • Walking/moving imbalance and a fear of falling 
  • Active or recent diabetic foot ulcer (utilize home-health about 85% of their days.) [1
  • Painful and annoying neuropathy impacting sleep, walking, cooking, and daily routines.

We serve patients with peripheral neuropathy, diabetic neuropathy, chemotherapy-induced peripheral neuropathy (CIPN), B12-deficiency neuropathy, and other causes of neuropathy. We bring fall-prevention, foot-health, and comfort-focused care right into your home - pairing neuropathy home nursing with nurse-supervised caregivers so you can stay safe, steady, and more independent every day. Our nurses and caregivers can provide: 


  • Nurse Assessments and Care Plans – We test all neuropathy symptoms to determine severity, track fall history, measure skin & foot status, understand pain/sleep, medications, provide specialized footwear/orthotics, and home layout. We then develop a personalized plan with clear goals (fewer falls, no new ulcers, better sleep, lower blood sugar) and design a simple daily checklist you and your care team can follow. 
  • Medication & Pain Management - Medication reminders/administration for diabetes and neuropathic pain regimens with side-effect and interaction monitoring. Comfort routines (heat/cold safety coaching for reduced sensation, pacing, relaxation, TENS coordination if ordered) to improve function and sleep. 
  • Foot Care & Ulcer Prevention - Daily foot/skin checks to catch redness, blisters, calluses, cracks, or drainage early. 
  • Fall-Risk Reduction, Balance and Mobility - Home safety upgrades, Transfer training & movement support, and Daily balance/mobility exercises coordinated with PT/OT plans. 
  • Activities of Daily Living Support - Bathing, dressing, and grooming with techniques that work around numbness, pain, and fatigue. Meal prep & hydration support with safe-handling strategies for reduced hand sensation. Household help (light laundry/tidying) to keep pathways clear and reduce trip hazards. 
  • Blood Sugar and Lifestyle Support – Blood sugar lowering routines tied to your clinician’s plan with foot-friendly activity and sleep hygiene coaching to reduce nighttime pain flares. 
  • Monitoring, Documentation and Early Escalation – Same-day nurse reviews for symptom tracking, vital/orthostatic checks, and red-flag checks to determine if there’s a need for escalation. We provide direct coordination with your PCP, endocrinology, oncology, podiatry, or wound care. 
  • Caregiver Education and Family Support - Hands-on teaching for daily foot checks, safe transfers, device use, and home exercises. We coordinate caregiver respite hours to prevent burnout and keep families healthy. 
  • Flexible Schedules - From a few hours per week to 24/7 care with overnight safety care. These are all supervised by an RN with routine check-ins and plan updates.

Out of the estimated 306,533 individuals with neuropathy in the counties and jurisdictions that we serve, around 134,874 have high-severity neuropathy.
