One Nurse Who Keeps The Whole Picture Together

When an aging parent has several doctors, a long medication list, and family who live out of state or can't step away from work, important details fall through the cracks. Our Registered Nurse Care Coordinator becomes the single point of contact who organizes care, talks to physicians and pharmacies, follows up on every recommendation, and keeps your family informed.

Care coordination can stand alone or be combined with our CNA personal care and medication management services. One team for doctors, medications, appointments, and daily support.

How It Works

In-home RN assessment. Every plan starts with a nursing assessment in the client's home covering current health concerns, medications, providers, recent hospital or ER visits, home safety, nutrition, memory and cognition, emotional and social needs, and the existing support system. The RN then recommends a care coordination plan built around the client's actual needs.

Attending doctor's appointments. Early on, the RN attends a primary care visit to introduce the care coordination role, get permission to communicate with the office, review medications and follow-ups, and bring the family's questions to the physician. We recommend the RN attend primary care visits regularly and key specialist visits when needed. Physicians value having a nurse who can report what is happening between visits, including medication use, vital signs, and changes in condition, so treatment decisions are based on complete information.

Meeting at the office keeps costs down. The RN typically meets the client at the doctor's office, so you are billed only for the nurse's time at the appointment, not travel. A CNA can provide transportation in the client's vehicle or accompany them by Uber or MetroAccess, or a family member can bring them. Routine visits like dental or podiatry usually don't require the RN.

RN wellness check-ins. Between appointments, the RN can visit for a basic head-to-toe nursing assessment, vital signs, a medication review, and a check on how well the client is managing the plan of care, reporting concerns to the physician or family as needed.

Medication oversight. The RN maintains an accurate medication list, follows up on changes, and communicates with the pharmacy. Full medication management, including pharmacy pickup, tray fills, and reminders, is available as an add-on service.

Keeping Families Informed

Your designated family member or representative receives timely updates according to the client's wishes: after important appointments, whenever there is a change or concern, and at agreed intervals. Updates can be a quick call, text, or email, a detailed follow-up when needed, or a monthly summary report. The goal is for families to stay informed without managing every detail themselves.

Flexible Services And Billing

Families may choose RN care coordination only, medication management, CNA services, or any combination. Services are billed separately but work together.

• RN Care Coordination: billed hourly on actual time used, including attending appointments (with waiting time), calls with physicians and pharmacies, family communication, medication coordination, and monthly reports. Call for current rates.
• CNA Personal Care: billed hourly with a four-hour minimum per visit. Includes bathing, dressing, grooming, toileting, meals, medication reminders, light housekeeping, laundry, errands, prescription pickup, companionship, standby assistance, and reporting changes to the RN. Call for current rates.

Services can be used weekly, monthly, every few months, or only when a specific need comes up, and the plan adjusts as needs change.

The RN provides assessment, education, communication, and coordination; the client and/or authorized representative remain responsible for healthcare decisions and for following the recommended plan.
